More than one person can nominate the same girl.
Please write a brief description of contributions made by the nominee in each of the categories below. The nominee does not have to make a contribution in all categories to qualify.

The top five girls will each receive a $500 scholarship. From the nominees the Selection Committee will choose several girls to participate in the Opening Ceremony. All the nominees will be listed in the Souvenir Program and will be included in a reception in their honor.
