Volleyball Festival 2012 - Housing Request Form

Please provide the below information in order to request accommodations for this year’s event. All teams outside a 50 mile radius of Phoenix must stay in approved housing. Parents, Friends and Family booking accommodations through the Festival will receive free admission to Opening Ceremonies at Chase Field and all playing venues. Teams must register on AES and pay the entry fee before accommodations are confirmed. After submitting your request, you will be contacted within 3 business days by a PSA Sports Personal Housing Coordinator to finalize your reservations.
