All teams must submit just the Festival Waiver Form for all registered Players, Coaches and Chaperones to the Volleyball Festival by May 10, 2013. If registering after this date, the form is due upon registration.
These forms should be submitted electronically via email (email@example.com) or fax (877-628-2804). Please limit submissions to only one Team per email or fax. If sending via email, scan all required documents to your PC and make sure to rename the file(s) so that it starts with the Team Name you registered with on AES. For example, a good naming convention would be: Epic 14-1 Waiver Forms.jpg. Preferred file formats are .jpg, .tif, or .pdf.
Additionally, the Festival Waiver Forms should be brought to onsite check-in along with copies of all registered Players' USAV Medical Release Forms.
A Team Coach or Chaperone must keep a copy of all paperwork in their possession at all times while at the Volleyball Festival.

The below forms are required of participating Foreign Teams. All forms should be submitted to the Festival office via email or fax per the above instructions by May 10, 2013. All forms should be brought to onsite check-in as well.
