FESTIVAL FIESTA CLASSIC REQUIRED FORMS:
No waiver forms are required for the Feb. 15-17, 2014 Festival Fiesta Classic. Teams must present verification of AAU membership at team check-in with one of the following:
VOLLEYBALL FESTIVAL REQUIRED FORMS:
All U.S. and Foreign teams must submit the Festival Waiver Form for all registered Players, Coaches and Chaperones to the Volleyball Festival by May 9, 2014. If registering after this date, the form is due upon registration.
These forms should be submitted electronically via email (email@example.com) or fax (877-628-2804). Please limit submissions to only one Team per email or fax. If sending via email, scan all required documents to your PC and make sure to rename the file(s) so that it starts with the Team Name you registered with on AES. For example, a good naming convention would be: Epic 14-1 Waiver Forms.jpg. Preferred file formats are .jpg, .tif, or .pdf.
The Festival Waiver Forms should be brought to onsite check-in along with copies of all registered Players' USAV Medical Release Forms.
A Team Coach or Chaperone must keep a copy of all paperwork in their possession at all times while at the Volleyball Festival.
ADDITIONAL FOREIGN TEAM FORMS:
In addition to the Festival Waiver Form and USAV Medical Release Form per the instructions above, all Foreign Teams must also mail in the below forms to AAU by May 9, 2014. If registering after this date, the form is due upon registration. The below forms and associated fees should be mailed to:

2. AAU Youth International Athlete Membership Application Form for all Players ($14 per player)
3. AAU International Non Athlete Membership Application Form for all Coaches and Chaperones ($37 per adult)
